SR22 Form

An SR22 form is the actual form filed with the state department of motor vehicles (DMV) for your SR22 insurance policy. The policy is actually named after this specific form (which has the same name in each state).

SR22 insurance policies are required for drunk driving offenders nationwide in order to reinstate your driving privileges after the DMV suspends your license when you're first arrested and subsequently convicted for drunk driving.

Before your driving privileges are restored you must seek out an insurance agency (or talk to your current insurance agency if they offer an SR22 form and get them to file the form with the state department of motor vehicles.

SR22 Form Description

The SR22 form is in essence an agreement between the insurance company and the DMV stating that they are providing the required coverage for you. The agreement also means that if for any reason at all they are not providing that coverage for you at any time they have to notify the DMV (usually through another SR form). Once the DMV receives this notice they will usually suspend your driving privileges for not maintaining the required high risk auto insurance coverage required for all drunk driving offenders.

So as you can see the SR22 form is basically the contract between the DMV and the insurance company. First you work out the deal with the insurance company (how much you'll pay for the specified amount of coverage, etc...) and then they'll file the SR22 form for you which lets the DMV know that they are now responsible for your insurance needs.
